Internal
Revenue Service: You will want to be aware
of at least two forms. (1) Form 1023 Application
for Recognition of Exemption and (2) Form
990EZ Short Form Return of Organization Exempt from
Income Tax. Both are available in a fill in
the blank pdf format which is downloadable from
www.irs.gov. Click the Charities and Nonprofits
tab, then the Forms and Publications
link, then the Form and Instruction Number
link. In addition to these forms, you should also
review Publication 526 Charitable Contributions
and Publication 561 Determining the Value of
Donated Property.
State of
Ohio: For other states you will typically
want to contact the Attorney General's office. If
you intend to solicit donations in Ohio you will
need to register using the form Charitable Organization
Registration Statement and you will need to
file form Annual Financial Report of Charitable
Organization each year. In lieu of the Annual
Report you can file your form 990. To obtain these
forms you can go to www.ag.state.oh.us. Click the
AG Forms tab, then the Charitable Law
link, then the link for the form you need.
Columbus
Charitable Application: Obviously, if you
are not from Columbus, OH, you will want to determine
if your city requires a permit to solicit donations.
Columbus does and many other larger cities will
